A printable symptom diary template (and an easier way to keep one)
Use the template below on paper, or print this page. It covers what doctors usually ask about: what you felt, how bad it was, what you took and what might have set it off. When paper gets hard, Heylixir keeps the same diary for you by voice or tap.

What to write down each day
Keep it simple so you can keep it up. For each entry, note the date and time, the symptom, how bad it was from 0 to 10, how you slept, your mood, anything you took and anything that might have set it off.
Do not worry about being perfect. A few honest lines on most days are far more useful than a perfect week followed by months of nothing.
Date, time, symptom and 0 to 10 severity

Meds taken and possible triggers

When paper gets hard
Paper diaries are great until a flare, a foggy day or sore hands get in the way. A voice note is a gentler way to keep the habit going. In Heylixir you say how you feel and it fills in your mood, symptoms, meds and journal, with Halo AI looking for patterns across weeks.
You can also create a clear summary to bring to appointments, so you do not have to carry a stack of pages.
